The Homeless Engagement and Response Team (HEaRT) works to reduce homelessness and strengthen healthy, safe communities through outreach, education, and compassionate support. By meeting people where they are, HEART stands alongside the most vulnerable and creates lasting, sustainable change. HEaRT serves our most vulnerable by meeting people where they are and fostering sustainable change within our community. 

 HEaRT provides the following services in Sutter and Yuba counties:

  • Provide street outreach to individuals and families who are experiencing homelessness by meeting folks where they are and identifying barriers to mainstream services.
  • Mobile Coordinated Entry (CE) assesses and connects people experiencing or at risk of homelessness to services throughout Sutter-Yuba Counties, mitigating the risks associated with being unsheltered, while building rapport.
  • Collaborates with community stakeholders in a continued effort to create innovated process' for this population and providing streamline approaches to treatment through our Start to Finish Program (STF).
