Case Management and Housing Navigation Team
Case Management- are the members responsible working on-on-one with the participants with all their needs. They assist with getting their vital records, CalFresh and Medi-Cal benefits, setting up primary care, and income. Case managers adapt to their clients' needs to ensure that their individual plan will successfully lead them to housing.
Housing Navigation- are the members that work directly with clients to get them housed. The housing navigators meet with each client and strategize which housing opportunity best suits them. They work closely with the community, landlords, and other agencies to secure successful housing.
Shelter Monitors- Our shelter monitors are a crucial part of our shelter's upkeep. They help participants and staff daily by ensuring client's needs are met safely. They are at the shelter 7 days a week including holidays. The monitors allow the participants to have access to a community room and their kitchen. They also are the individuals who flip units and do intake appointments with the clients.
Peer Mentor- are the advocates for the participants. The Peer Mentor’s primary duty is to encourage participants of the county's homeless programs to utilize services available in the community, to support their path of self-sufficiency. They work with case manager and housing navigators to help determine client's needs.
HEART Team- are the members of our team that go in the field to recruit and support our communities unhoused individuals. Their objective is connecting individuals and families with appropriate resources and reduce the risks associated with homeless. They meet individuals where they are and strive to create a better community with the individuals with support of community partnerships.
